Before and After Photos of Revision Rhinoplasty with Autologous Rib Cartilage: Using Autologous Rib Cartilage and Rib Particles > O.P Column

Hello, this is Dr. Kim Hyung-taek from Kowon Plastic Surgery Clinic.

Today, I’d like to share a case of a revision rhinoplasty.

This is quite a complex case where the patient had undergone surgery about seven years ago using donor rib cartilage, ear cartilage, and silicone.

Last year, they had a reconstruction surgery elsewhere, but were dissatisfied with the results and came to us for a revision.

In the previous surgery, there was an attempt to reduce a dorsal hump, but the results were unsatisfactory and uneven.

The nose appeared somewhat short, and there were irregularities along the middle of the nasal bridge, resulting in an unnatural appearance and slight asymmetry in the nostrils.

For this third surgery, considering the history of using donor rib cartilage, we recommended using autologous rib cartilage.

The patient wanted a more refined look, so we decided to narrow the nose by performing an osteotomy on the dorsal hump and bridge. Instead of silicone, we used rib particles for the nasal bridge.

From the frontal view, you can see that the nasal bridge has been significantly narrowed.

From the base view, the nasal tip has been elevated significantly, giving a more slender appearance as desired by the patient.

From the side view, the nose now has a straight, refined line from the glabella to the tip, with the previous indentation along the middle of the bridge eliminated.

The nasolabial angle has also been notably improved.

It’s important to note that the nasal bridge may initially appear slightly higher because autologous rib particles, like other autologous tissues, can absorb about 10-20% over time.

Therefore, the bridge was set a bit higher initially, and after about six months, it will naturally settle to a more refined and natural appearance.
