Today, I’d like to introduce a case of a male patient with a bulbous nose. He had a very low and broad nose, especially prominent when viewed from the front.
The patient also had thick skin and wide nostrils, with a low nasal bridge overall, particularly between the eyes.
To address this, we used a 4mm silicone implant to achieve sufficient height and used septal cartilage and ear cartilage to shape the nose.
We also performed a depressor septi muscle resection to prevent the nose tip from drooping.
Additionally, there was a slight dorsal hump on the nasal bridge, so we performed a hump removal procedure as well.
Here’s a view from below. The wide, spread-out nose has been reshaped to a more refined and elevated appearance.
We combined a nasal tip cartilage binding technique with a non-incisional alar base reduction.
As you can see, the non-incisional technique leaves minimal visible scarring.
You can clearly see how the impression changes when the previously broad nose is brought together.
For men with short, bulbous noses and thick skin, the key to a successful surgery is how well the nose tip can be extended and firmed up.
If the skin is too thick or the pulling force is strong, sometimes the nose may shorten a bit compared to the initial result.
However, this can usually be corrected by adding an additional piece of cartilage to restore the desired length.
