Today, I’d like to introduce a case involving a male patient with a bulbous, broad, and low-set nose, particularly noticeable when viewed from the front.
The patient presented with thick skin, wide nostrils, and a low nasal bridge, especially between the eyes, giving the overall nose a flat and heavy appearance.
To improve the nasal height and contour, we used a 4mm silicone implant to elevate the bridge. Additionally, septal cartilage and ear cartilage were used to structure and refine the nasal tip.
To address the drooping of the tip, we performed a depressor septi muscle resection, which helps prevent the tip from pulling downward, particularly when smiling.
There was also a slight dorsal hump on the nasal bridge, which we corrected through a hump removal procedure.
Here’s a basal (bottom-up) view after surgery. The previously wide and spread nasal tip has been reshaped into a more defined and elevated form.
We used a combination of nasal tip cartilage binding and a non-incisional alar base reduction. This method is especially beneficial as it allows for nostril narrowing with minimal visible scarring.
The result is a significantly refined nasal shape, especially when compared to the broad, rounded tip before surgery.
In male patients with short, bulbous noses and thick skin, the success of the surgery depends largely on how effectively the nasal tip can be extended and reinforced.
In some cases, if the skin tension is strong or the tissue is too thick, there may be some degree of shortening after healing. When this happens, the nose can usually be re-extended by adding an additional piece of cartilage to restore the desired projection.
